My favorite Character Questionnaire is by Writer's Digest. It was published in their "Write a Novel in 30 days" edition last year. The online link is here:

internal conflicts: Feels that she is never good enough for anyone. Caused by a past relationship that left her a single parent.
external conflicts: Custody dispute over 2yro daughter
occupation/education: Performer (dance) - Associates Degree w/partial to Bachelors of Performing Arts.
