I hesitated in responding to this form, in fact, started it and stopped, then returned later. You hit on so many essential points. I did, however, make one suggestion. Probably more responses might be gained if all respondents elected the Anonymous option. I didn't, but I thought about it.

Also, on a few questions I thought there should be more options. Domestic violence, like any abuse, has multiple levels and ramifications. Of course, too many options could make a survey unwieldy. It's hard to know what would be optimal.
