If they are puzzled by the review then there's no reason why they can't reply to the review and ask for clarification. The main thing, of course, is staying polite when doing so.

Though.... if you didn't understand what you were reading and yet you reviewed it anyway, then you should as a reviewer acknowledge that (or just not review!). If you didn't know that you didn't understand..... then maybe that's on the writer for not being clear?????

Two sides to every story and both parties should be ready to communicate.
