Unfortunately, I have been unable to find an Afrikaans poetry form which matches the pattern you provided. The closest I could come is the Rondel Prime ( http://thepoetsgarret.com/2019Challenge/form08.html ), which could be extended into a Nonce form ( http://www.baymoon.com/~ariadne/form/self.created.form.htm ) for some specific occasion.
